Is the information I enter on the web site safe?
The web site is located on secure servers and requires your unique user name (e-mail) and a password selected by you in order to enter/view your data. Critical Medical Information®, LLC (CMI) makes every effort to assure your data is secure. However, it is your responsibility to maintain the security of your password. CMI cannot be responsible for your data being entered or changed by person(s) who have obtained your password. (Please see Terms of Use)
Is the information on my flash drive safe?
The information on your flash drive is downloaded directly from the CMI web site. You are given the option when you complete the Personal Contact Info section on the web site to password protect your flash drive. You will be given the option to choose "Yes" or "No." If you select "Yes" you will be prompted to fill in a password. You are strongly urged NOT to use the same password for your flash drive(s) that you use for access to the secure web site. Password protection on your flash drive(s) gives a certain level of protection, but it is not foolproof if your drive is lost and falls into the hands of a hacker. If you choose not to password protect your flash drive it will remove one impediment to Emergency Medical Personnel getting access to your information. Only you can decide the level of security you want on your flash drive. You can change the flash drive(s)password by resetting it in the Personal Contact Info section of the web site and downloading the information to your flash drive(s).
What is the Customer Serial Number (CSN)?
The Customer Serial Number comes packaged with your CMI flash drive or is given to you when you purchase the CMI service. The CSN certifies you are a legitimate Critical Medical Information® member and authorized to log onto the site. Once you are admitted to the site, before doing anything else, you must select the menu option Personal Contact Info and complete all of the required fields (in bold) in that section. The CSN is located on the package insert of your CMI flash drive(s) or it will be in the body of the text establishing your service if you have chosen not to use a CMI flash drive.
What about my social security number?
Critical Medical Information®, LLC advises you NOT to enter your social security number anywhere on the system. However, your SS# may be part of your medical insurance information. If that is the case we strongly advise you indicate only the last four numbers of your SS# so your insurance carrier can identify you, but you will not divulge your full SS#. The final decision is your responsibility. CMI cannot be held liable if you chose to divulge your SS# in your records and it is compromised.
What happens if I lose my flash drive?
If you lose your flash drive all of your information is still available on the web site and accessible with your user name and password or with the Emergency Access password. However, the information on your flash drive that is not password protected is available to anyone who finds your flash drive. It would also be prudent to go to the web site and change your password for web site access in the Personal Contact Section. If you lose your flash drive you can order a replacement drive(s) the CMI web site.
Password
How do I establish/manage my passwords?
Your passwords are created and managed under the Personal Contact Info menu item on the web site. We suggest that you use passwords (particularly the password associated with your user name and which permits read/write access to the web site) that are a random combination of letters and numbers. When you initially log onto the web site the Personal Contact Info must be the first item you complete. You will be required to give your user name (your primary e-mail address) and create and verify a password for read/write access to your records on the web site. In addition to the password linked to your user name, you will be required to add a password that allows medical personnel to have read only access to your medical information on the web site. You will also be asked to select if you want your flash drive(s) protected with a password. If you select "No" no further action will be required. If you select "Yes" to protect your flash drive with a password you will be required to provide and verify a password for your flash drive. The flash drive password will not be active on your flash drive until you download your data to the flash drive. You are strongly urged to use different passwords for your web site log on, your web site read only Emergency Access and for your flash drive(s). Your Web Emergency Access password and flash drive password, if any, are printed on your wallet card. If you elect to not to password protect your flash drive it will eliminate the step of emergency personnel having to obtain the Emergency Access password from you or your primary emergency contact.
What if I forget my password for the web site?
If you enter the wrong password you will be challenged. If you cannot remember your password click on the forgot password beside the password cell and a replacement password will be sent to your e-mail address of record. If you are still unable to gain access to your file please e-mail - support@criticalmedicalinformation.com giving your full name and you will be contacted and assisted with your logon.
What if I forget my password for my flash drive(s)?
Your flash drive password is printed on your wallet card. You can also log on to the web site and change your password in the Personal Contact Info section. If you change your password you must download the information from the web site to your flash drive(s).
What if I forget my password for read only Emergency Access to my web site?
You can log on to the web site and change your password in the Personal Contact Info section. If you change your password you must notify your Primary Emergency Contact person to alert them to the new password.
Access
What if I get to the doctor's office or hospital and don't have my flash drive?
All of the information on your flash drive is on the web site. If medical personnel have access to a web-enabled computer and you or your representative give them your web site Emergency Access password they will have read only access to your information over the Internet.
Why should I carry the wallet card?
You will be able to print a wallet card containing your contact information, contact information on your primary emergency contact, blood type, permission to treat and emergency access passwords for your flash drive and web site. Your Wallet Card should be reprinted any time you change the password for your flash drive(s) or web site, if you change primary emergency contact or if it becomes worn.
How do I register on the web and enter my data?
- Make sure PC is turned on and connected to the Web
- Insert MyCMI+™ into USB port. Cancel if it attempts to auto launch

- Enter Customer Serial Number found on these instructions and click "Submit"
- Click on "Continue Registration" icon
- Complete registration form (bold items required)
- Do not confirm your e-mail at this time
- Enter passwords and record them in a safe place for future reference
- Click "Save and Submit"
- Click "Edit" on Personal Contact Info, go down to User Name & Password and click on "click here to confirm." This initiates process to authenticate your e-mail address to protect your data
- Close confirmation message.

How do I register and logon using my flash drive?

Can I create more than one Living Will or Power of Attorney for Health Care?
Yes. We realize that you may change your state of residence or be making an extended visit to a state other than your state of residence. If you are concerned your Living Will or Power of Attorney will not be recognized in the new state you can create a Living Will and Power of Attorney for the state you will be visiting. You will need to reenter and re-execute your Living Will and Power of Attorney and download them to your flash drive each time you change them. You will NOT need to redo your written Living Will and Power of Attorney unless you have made changes to them on your site since they were first created.
Executed versions of your advance directives can be scanned into your MyCMI+™ and stored under Medical & Legal Images. When printed they are legally recognized as "facsimiles" that will be recognized and acted on by medical personnel.
I logged onto the site, but later I was automatically logged out and had to log back in, why?
After thirty (30) minutes without any keyboard activity on your part your connection to the Critical Medical Information® is automatically terminated to protect your information. The timer starts from your last keystroke so it should not produce a problem in normal usage situations. If you are automatically logged out you will have to reenter any information entered after your last save before being automatically logged out.
Membership
How long does my subscription to Critical Medical Information®, LLC last?
Your initial subscription lasts for one year from your initial log on to the site. However, you can renew your subscription annually for a small fee that insures your data will continue to be stored on the web site and that you can continue to update your data and flash drive(s) whenever your information changes. As a member you will also be able to use any new standard features added to the site. To renew your subscription go CMI's web site (www.criticalmedicalinformation.com) and select ORDER MyCMI+™.
What happens if my subscription expires?
Two weeks prior to your subscription expiration you will receive an e-mail notice to your e-mail address of record with CMI. You will be told your subscription expires in two weeks and given a link to the site where you can renew. On the expiration date of your subscription you will receive another e-mail notice that your subscription has expired and again give you an option to renew. If you have not renewed your subscription within two weeks of its expiration all of your data stored on the site will be removed and archived. Access to your data after that point will require reinstitution of your membership and additional charges for retrieval of your information from archives and reinstalling it on the web site. Memberships not renewed within twelve months of their expiration will result in permanent deletion of the data.
Why should I renew my subscription?
The information and records you place on your file on the CMI web site can be a permanent repository for that important information. Renewal of your subscription assures that information and the effort that went into collecting it will be maintained. It also allows you to continue to make changes and additions to your information and download to your flash drive(s). Also members have full access to any improvements made to the service.
What is a trial subscription?
You have the option of trying My CMI+™ for up to seven days. You will have the ability to use all of the functionality of the system except the ability to download to a flash drive and print Living Wills and Durable Power of Attorney for Health Care. At the end of the seven day trial you can purchase your membership and all of the data you entered during the trial will be transferred to your new membership and you will be able to fully utilize the site. At the time you convert your membership you will also be able to purchase the flash drive(s) of your choice.
Flash Drive
How can I be sure that the information I have entered on the web site is transferred to my flash drive(s)?
At the conclusion of entering/editing data on your site you have to click on the Download to Flash Drive menu item and follow directions (see below) to insure your additions/edits are downloaded. You can also add to or edit your information online when your flash drive is not available. The next time you logon you will be reminded at the bottom of the Member Table of Contents page that you need to download to your flash drive(s). If you have more than one flash drive you must download the data to each of the flash drives every time you make a change.
After downloading to flash drive the flash drive MUST be extracted (see directions under Download to Flash Drive) and the drive must then be launched to insure the download was successful.
How can I download to my flash drive(s)?
- Make sure your computer is connected to the internet and insert your flash drive in a USB port. If your flash drive tries to launch just click cancel. Then open your browser (Internet Explorer, Firefox, etc.) and log on to www.criticalmedicalinformation.com and click on Member Login.
- In the dialogue box that comes up enter your user name (e-mail address) and the password you entered to allow you to log on as a member. If you forgot your password indicate that and a new one will be sent to you if you click on the forgot password link.
- Once you are logged in click on the menu selection "Download to Flash Drive" and you will go to page where you can download your information. Click on the "Download My Data" icon and then click on "save file" in the dialogue box that appears. Another dialogue box will appear.
- Depending on how your computer is configured you will either be confronted with a list of drives on your computer or you will need to locate the Critical Medical Information drive. If you need to locate your flash drive click on the "down arrow" button at the end of the "Save in" box. When you click there you should be presented with list of drives, one of which will be "MyCMI." Click on that icon and click "Save" to download to your flash drive. Do NOT change the name of the file (member) when downloading.
- Your data is now downloaded to your flash drive. Close the "Downloads" dialogue box if it appeared. Now go to the Safely Remove Hardware" icon (lower right corner of your screen shaped like a gray box with a green arrow over it) and click on your MyCMI drive. You can now remove your drive. Please do so.
- Reinsert your MyCMI+ flash drive in a USB port. It will launch a dialogue box, select and click on Launch MyCMI+. That will open a dialogue box with a blond female medical person on the right and three icons (Emergency Access, View Member Data, and Member Login) on the left.
- Select "Emergency Access." That will launch your flash drive and a dialogue box will ask you to wait while it is loading.
- You will then be presented with your MyCMI+ menu. If you have NOT password protected your flash drive your data will be displayed. If you HAVE password protected your flash drive you will be challenged to enter the password you assigned to your flash drive. Your data will then be available. If you have protected your MyCMI+ flash drive you may want to review the help icons in the Edit section of your Personal Contact Info the next time you are logged into your site to learn the pros and cons of password protecting your flash drive.
- Your data is now confirmed to be downloaded to your flash drive.
How many flash drives can I have?
Your lifestyle (play sports, swim, ride bikes, etc.) may make it convenient for you to have more than one flash drive. You may order additional flash drives by going to CMI's web site (www.criticalmedicalinformation.com) and select ORDER MyCMI+™.

Using flash drives not purchased from CMI.
You can purchase the service only from CMI to use on your own flash drive. However, using flash drives not purchased from CMI introduces limitations you may wish to avoid.
All flash drives sold by CMI are clearly marked with the Critical Medical Information logo that helps emergency personnel recognize the drive contains medical information. Your wallet card instructs emergency personnel to look for a flash drive with the CMI logo. To identify your personal drive as containing important medical information you can purchase a CMI logo sticker to put on your drive by going to CMI's web site (www.criticalmedicalinformation.com) and select ORDER MyCMI+™.
All flash drives purchased from CMI have auto launch capabilities and CMI proprietary software. Non-CMI flash drives MUST download this software by initializing their drive under the Download to Flash Drive menu option on the web site. Failure to initialize a non-CMI drive will make it impossible to download and access your information on the flash drive.
CMI will sell our service without our flash drive(s). However, because of the issues outlined above we suggest you seriously consider buying the service with the CMI flash drive(s) if you are not very comfortable using computer technology and/or you want the CMI logo embossed on your drives.
My flash drive won't work.
Be sure it is inserted correctly into the USB port on your PC computer (MyCMI+™ is currently not compatible with Apple computers). Make sure your computer is turned on. Make sure your flash drive hasn't been damaged through shock (dropped), exposed to moisture or otherwise damaged. If your flash drive will not operate you can attempt to restore it by going to the Restor option under the Download to Flash Drive Menu. If the drive cannot be restored you can order a replacement drive by going to CMI's web site (www.criticalmedicalinformation.com) and select ORDER MyCMI+™. Because of the variety of environmental conditions your flash drive might be exposed to CMI cannot assure or guarantee their performance after initial delivery. The manufacturer does guarantee the flash drives against defects in manufacture for a period of five years. Any claims under this warranty will be honored at the sole discretion of CMI.

Why can it take a long time to download data to my flash drive(s)?
Whenever you download information from your CMI web site to your flash drive it downloads all information on your site, not just the changes/edits you have made. This insures you will have a seamless update to your flash drive and all information is loaded.
There are many factors that influence the time it takes to download data from your computer to your flash drive. The type of connection you have to the Internet (modem, broadband, etc.) will significantly effect download times. You may have an older computer with a 1.0 USB port rather than the newer 2.0 USB port. This can materially slow download speed. You may have a lot of data stored in your records and this can slow download times. If you have stored a number of images this can significantly slow download times. Whenever you initiate a download to your flash drive it is essential that you not interrupt the download process or it may cause your flash drive not to operate properly resulting in inability to access your information. If the download is interrupted for any reason you must reinitiate the download.

Can I make multiple additions or changes to my data on my web site?
You can make changes or additions to information on your web site as often as you wish. Just remember to download to your flash drive(s).
May I enter/change data on the web site without having my flash drive connected to the computer?
