Fill in blanks and check radio buttons to record your information. You have access 24 hours a day, 7 days a week to enter, edit or view your information. Downloading information to your flash drive(s) or printing it is easy and menu driven.

Before a doctor visit generate and print a report for you or your family member listing all doctor visits between two dates you select plus all of your medications, allergies, etc. Also provides space for your questions and your doctor's instructions.

Advance Directives
With MyCMI+™ you can create a Living Will and Durable Power of Attorney for Health Care for each of the 50 states and DC. This ensures your wishes will be known during your medical treatment.

Missing Person Flyer
If a loved one is lost or missing you can quickly create a missing person flyer on any web enabled computer. It can be printed and shared with police/security on the scene and electronically transmitted to surrounding police jurisdictions, Amber Alert, etc.

Access:
Your information is always available on the web or on your flash drive(s). Your data is protected on a secure web site with your unique user name and password and only you can enter or edit your data. Emergency responders and medical personnel can be given "view only" access to your flash drive or web site with passwords you provide.

Use:
Changing or adding to your information is simple and intuitive. Just log onto the secure web site using your unique user name and password, select from the menu, add or edit information and click to save. Flash drive contains all of your information in a "view only" format and can be password protected.

Store:
Information on the web site is automatically stored every time you save. Your information can easily be saved to your flash drive by selecting "Download to Flash Drive" on your menu and following the instructions.
